Monitoring and Support Groups
The privacy notice should be supplied to any party offering to act as a member of a Monitoring and Support group. The confidentiality agreement should be signed prior to engagement with the group and returned to the District Safeguarding Officer. July 2022.
This form should be supplied to all those about whom a safeguarding concern has been raised and as a result are now engaging in a Monitoring and Support Group. It contains a GDPR compliant privacy notice and an information receipt. July 2022.
Note regarding 2022 forms update:
The update has ensured that all forms are UK GDPR compliant and is also following the update to our Privacy Notice, which can be found at Privacy notice (methodist.org.uk).
An Annex 3 has been added at the bottom of the Privacy Notice in respect of the additional data that the safeguarding team processes. This means that the forms are now shorter and more user friendly and that we do not have any more than one main overarching Privacy Notice. In line with this we have renamed the updated safeguarding Privacy Notice forms to Personal Data Processing Statements. It signposts to our main privacy notice which contains all of the detailed information as to how we collect, process and retain data.
